Requesting Your Health Care Records

You have the right to request a copy of your health care records maintained by our office.

To request your records:

1. Complete a Medical Records Request Form. You may obtain this form by contacting our office.

2. Submit the completed and signed form to our office by: Patient online portal or Secure email (if available)

3. Please provide sufficient information to identify your records, including your full name, date of birth, and the records you are requesting.

4. If you would like your records sent to another person or health care provider, please include a signed written authorization identifying the recipient.

5. We may request proof of identity before releasing your records to protect your privacy.

6. If our office maintains your records in an electronic health record system capable of fulfilling your request electronically, and you request an electronic copy, we will provide your records in electronic form as required by applicable Texas and federal law unless you agree to receive them in another format.

7. Any fees permitted by applicable law will be communicated to you before your records are released.
